The NACC Uniform Data Set version 4 (UDSv4) is a standardized longitudinal clinical research dataset used by Alzheimer’s Disease Research Centers to collect annual participant-level information on cognition, diagnosis, medical history, function, behavior, and demographics. Introduced in 2025, UDSv4 reflects current scientific and operational needs in AD/ADRD research by adding new forms and updated measures, including social determinants of health, disease-modifying treatments, improved demographic items, Spanish-language materials, and enhanced electronic data capture. The system is designed to support harmonized multi-center research, improve data quality and accessibility, and facilitate downstream analysis through modern submission, validation, and search tools
The Uniform Data Set version 4 (UDSv4) is NACC’s standardized, longitudinal data collection framework used across NIA-funded Alzheimer’s Disease Research Centers (ADRCs) in the United States. It captures annual clinical, cognitive, demographic, functional, and phenotypic information from participants ranging from cognitively normal individuals to people with mild cognitive impairment and dementia. Released in 2025, UDSv4 modernizes the previous UDS by updating content, improving accessibility, and expanding coverage of emerging concepts in Alzheimer’s disease and related dementias (AD/ADRD) research.
